Complete ALL SECTIONS of the registration form below before moving on to sign release forms (Step 2) and to pay for your ticket (Step 3)
STEP 2: SIGN RELEASE FORMS
Please use this 2024 Senior Release Form link to go to a folder where you can download and sign the 2023 Senior Release Forms. If you indicate on this form that your student has a life-threatening allergy or situation, please also download and complete the Emergency Care Plan from the same folder. Please return all signed forms to the VHS office by May 1st. If you cannot access the folder, please email [email protected] and we will email you the forms or you can pick up a copy of the form in the VHS office. All students must have completed forms on file before they load the buses for the event on Graduation Night.
